Simple interrupted suturing is a basic technique used to close wounds in surgery and medical procedures. Here's how it's typically done:
Preparation: Ensure the wound edges are clean, debrided, and properly aligned before starting the suturing process.
Needle and Thread Selection: Choose an appropriate needle and suture material based on the type and location of the wound. Commonly used suture materials include absorbable materials like polyglactin (e.g., Vicryl) or non-absorbable materials like nylon.
Starting the Suture: Begin by inserting the needle through one side of the wound edge, approximately 3-5 mm from the wound margin and at a 90-degree angle to the skin surface. Pull the needle through until approximately half of the suture material remains on one side.
First Bite: Insert the needle across the wound, penetrating the opposite wound edge at the same distance from the wound margin and at a similar depth. This creates the first "bite" of the suture.
Tying Off: Once the first bite is completed, tie off the suture by bringing the ends of the suture material together and securing them with knots. This brings the wound edges together, closing the wound.
Trimming Excess Material: After tying off the suture, trim any excess suture material to ensure a neat and tidy closure.
Repeat: Depending on the length of the wound and the desired level of closure, the process may be repeated with additional sutures spaced evenly along the wound length. Each suture is placed independently of the others, hence the term "interrupted."
Simple interrupted sutures are versatile and widely used for closing wounds of various sizes and complexities. They allow for precise alignment of wound edges, which is crucial for optimal wound healing and cosmetic outcomes. Additionally, they provide good wound edge eversion, which promotes healing and reduces the risk of wound complications such as dehiscence or infection.

      Hi, ejection fraction per se is not a contraindication for this surgery. Whether he's fit or not otherwise is decided based on clinical assessment. So, if the doctor seeing him assess that he's fit for surgery and chemo, then, port can be placed.

      Hi, Epidural provides prolonged pain relief and duration of pain relief is much longer. Remeber that epidural is analgesia whereas spinal is anesthesia. Also, epidural is more controlled and patients can even walk with the so called "walking epidurals" basically less drugs in less concentration. All this is not possible with spinal.

    Hello Doctor. Excuse my ignorance. I'm reviewing the results of the gastroscopy I had yesterday, and one of the things it says is, Wound Classification: Contaminated. Is that normal in this type of procedures? or maybe they used equipment that was not previously sterilized?

      Hi, gastroscopy usually does not use a wound classification as per our faculty practice. If it's a norm in your country or state, it would be best to discuss with the doctor who's performed the procedure to understand the meaning of the report.

I am scheculed for a Whipple due due to a Mucinous Cyst with worrisome features. Two questions: What are the advantages of "Open" versus a "Laparoscopic" Whipple? In your experience, what % of patients will need to be on insulin post surgery. Thank you for your input!

      A surgeon has to decide the technical approach ( open, laparoscopic or robotic) based on the disease extent, anatomical details, patient clinical evaluation and many such factors. If all these are taken into consideration, each approach has it's merit which depends on the case at hand. Insulin requirement usually is not an issue with Whipple if rest of the pancreas is not atrophic and functional before surgery. Hope that helps. Best wishes for your surgery.

      Hi, you do not need grounding pad if you're using only bipolar energy.
